of STRIDE patients and employees, any individual entering a STRIDE facility
must wear a mask or facial covering at all times.
We recognize the importance of our patients staying connected to their STRIDE providers for their health concerns during this pandemic. Because of the increased risk of exposure to the COVID-19 virus with any venture into public areas, we are limiting
clinic visits to only those who absolutely must see their care team in person.
We now offer telephonic and televideo visits so that patients can continue to get their health care needs addressed while decreasing their exposure to the virus during this time.

COVID-19 Public Testing will be CLOSED on Saturday, May 23rd, and Monday, May 25th, in observance of Memorial Day.
COVID-19 Public Swab Testing is available to the following individuals:
Any individual who has COVID-19 symptoms
Any individual who is employed as a health care worker, nursing home worker, or first responder
COVID-19 Antibody Testing is also available to:
Any individual who has had at least 14 days since the onset of symptoms or who has never had symptoms
Whether a person is on Medicaid, Medicare, private insurance or has no health insurance at all, cost is not a barrier to testing in Colorado. Thanks to executive action taken by the Governor’s Office, Colorado-regulated insurance plans are prohibited
from charging copays for testing, and Medicaid patients can also get tested without cost.

STRIDE CHC accepts Medicare Fee-For-Service, Health First Colorado Basic (Medicaid), CHP+ State Managed Care Network (SMCN), Colorado Access CHP+, and some private insurance. For individuals who are uninsured, there is a sliding fee scale for services.
STRIDE CHC also accepts the Colorado Indigent Care Program (CICP).

addressed, please submit using our Patient Concerns Form or by calling
patient who is dissatisfied with the care they receive can report a concern.
Filing a concern will not result in any adverse action or negative treatment.
If you have any concerns that cannot be resolved with your care team, please
